Current indications are that this is likely to be a defect in Mac OS with a very low rate of occurrence. We'd be very interested to hear if it turns out to be a problem that you can reproduce again with Fusion... And if you do reproduce it, please also consider bopping that Send to Apple button so that Apple gets a direct report of the issue.
